—A common complication of diabetes mellitus is ketoacidosis which results from the accumulation of acetoacetic and B-Hydroxybutyric acids. A less often seen and recognized metabolic alteration is diabetic ketoacidosis with alkalemia This disorder is a mixed acid-base disorder in which diabetic ketoacidosis exists alongside a state of metabolic alkalosis. Blood pH will be determined by which disorder predominates. Diuretic use, vomiting, alkali ingestion, and hypercortisolism have all been implicated as causes of this disorder.
