Acute myocardial infarction is a potentially life-threatening disease that must be diagnosed early and accurately at the emergency department. Its diagnosis may be difficult if manifested solely as a severe explosive headache in a young, nondiabetic, and nonobese man. We report a case of ST-elevation myocardial infarction that was initially misdiagnosed as a case of headache until routine electrocardiography was obtained.
